We are committed to protecting the personal and health information you share with us as part of our healthcare navigation and advocacy services, and to using it only as described in this notice or as permitted by law.
The information we hold
In the course of helping you or your family navigate healthcare, we may collect information such as your name and contact details, the names of your providers, appointment and scheduling details, insurance and coverage information you choose to share, and health-related information you or an authorized family member provide so we can assist you. We collect only what we need to provide our services.
How we may use and disclose your health information
We may use and disclose your health information for the following purposes: - For your care and coordination. To coordinate and provide navigation and advocacy support, including communicating on your behalf with your providers, facilities and others involved in your care. - For our operations. To run and improve our services, including quality review, training and administration. - To communicate with you and authorized family. To reach you and, with your permission, to share information with family members or others you authorize. - With business associates. With vendors who help us operate (for example, secure scheduling, communication or record-keeping tools) under written agreements that require them to protect your information. - As required or permitted by law. When we are legally required to disclose information, or to prevent or lessen a serious threat to health or safety.
We do not sell your information, and we do not use it for marketing or advertising. Other uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke at any time.

You have the right to: - Access and copy the health information we hold about you, and request it in a specific format. - Request a correction to information you believe is incorrect or incomplete. - Request restrictions on how we use or share your information. - Request confidential communications — for example, that we contact you only at a certain phone number or address. - Receive an accounting of certain disclosures we have made of your information. - Receive a paper copy of this notice at any time, even if you agreed to receive it electronically. - Withdraw an authorization you previously gave, in writing, going forward.
